Two bodies were swapped at the morgue of ULS São José, in Lisbon, and the error was only detected by one of the families during the wake. The body swap occurred in the local health unit responsible for that area of the Portuguese capital.
The Health Regulatory Authority considered this failure as being of "special gravity". The regulator identified deficiencies in the corpse identification procedures practiced in the institution.
As a corrective measure, the Health Regulatory Authority determined that ULS São José reinforce the corpse identification procedures. The objective is to prevent similar situations from occurring again in that or other health units.
