The Northeast Local Health Unit (ULS) reinforced its proximity services in the municipalities of Mogadouro and Freixo de Espada à Cinta from September 1st, covering more than 10,900 users. The reorganization integrates the Personalised Healthcare Unit of Freixo de Espada à Cinta into the Trindade Coelho Family Health Unit, which will now provide care to approximately 8,195 users in Mogadouro and 2,730 in Freixo de Espada à Cinta.
The Family Health Unit will now operate with two care facilities, maintaining services in both municipalities. In parallel, the Lagos do Sabor e Douro Community Care Unit is expanding its intervention to cover the entire territory of both municipalities.
Among the new services is an Integrated Continuing Care Team, aimed primarily at dependent people, those with chronic illness, palliative care needs, or difficulty traveling to health services. The team begins operations with capacity for 15 home beds, which can expand to 25 under a national pilot project.
Users will have access, at home, to medical and nursing care, rehabilitation, and psychological and social support. The measure aims to allow users with ongoing needs to be cared for at home, helping to avoid travel and hospitalizations that could be prevented.
