The origin of the accident was a crack in a weld between two steel bars that were 30 years apart in age. This defective weld eventually gave way.

With the rupture of the weld, the newer bar detached from its anchors and violently rotated outward, causing the train to derail.

The incident remains under investigation to determine all the circumstances that led to the accident.
