UBI discovers new treatment for stroke

The University of Beira Interior, through the Faculty of Health Sciences, developed a new treatment for stroke that reduces brain lesions and accelerates motor recovery in animal models. Stroke is one of the main causes of mortality and permanent disability in Portugal, and this discovery was led by researcher Sara Meirinho, in collaboration with specialists from Spain and the United States of America, integrated in the RISE-Health research unit.

The treatment is based on nasal application of nestorone, a molecule already approved for contraceptive purposes, adapted for the acute phase of ischemic stroke. This administration route bypasses the blood-brain barrier and hepatic degradation, allowing the drug to reach directly the affected nervous tissue. Pre-clinical laboratory tests demonstrated that a single dose administered shortly after the ischemic episode substantially limits the extent of neuronal lesions, through an anti-inflammatory action that modulates glial cells.

The results published in the specialized journal Drug Delivery and Translational Research confirm that the treated animal models recovered motor capacity more rapidly, opening prospects for attenuating permanent motor sequelae. However, the team remains cautious, noting that the treatment does not eliminate the underlying causes of the pathology, but protects the brain structure.

The study is still in the pre-clinical phase, with new toxicology studies and controlled clinical trials needed before advancing to human testing. In addition to stroke, the researchers point out that this administration route and neuroprotective effect may be applied in the treatment of other diseases of the central nervous system, including multiple sclerosis. The consortium is now preparing for regulatory validation of the technology and intends to secure community funding to transfer this solution to hospital practice.

O acidente vascular cerebral represents uma das maiores causas de mortalidade e incapacidade permanente em Portugal. Por conseguinte, a procura de respostas médicas eficazes mobiliza a comunidade científica internacional. A Faculdade de Ciências da Saúde da Universidade da Beira Interior (UBI) assumiu a liderança deste consórcio. A iniciativa integra investigadores da unidade RISE-Health, em estreita cooperação com especialistas de Espanha e dos Estados Unidos da América.

Como funciona o novo tratamento para AVC

O projecto de investigação clínica partiu de uma molécula já aprovada para utilização médica com fins contraceptivos. No entanto, os cientistas encontraram um mecanismo inédito para aplicar a nestorona na fase aguda do AVC isquémico. A administração da substância realiza-se por via nasal. Esta via inovadora contorna a barreira hematoencefálica e a degradação hepática, permitindo que a solução atinja directamente o tecido nervoso afectado.

Os ensaios laboratoriais pré-clínicos demonstraram resultados promissores. Com efeito, a administração de uma dose única pouco tempo após o episódio isquémico limita de forma substancial a extensão das lesões neuronais. Além disso, o fármaco apresenta uma acção anti-inflamatória determinante através da modulação das células gliais. Esta resposta biológica trava a degradação dos tecidos nas horas subsequentes à ocorrência do ataque.

Capacidade motora regressa mais depressa

Os modelos animais submetidos ao protocolo registaram ganhos funcionais significativos. Deste modo, os testes comprovaram que a capacidade motora regressa mais depressa nos indivíduos tratados com esta formulação nasal. Assim, abrem-se perspectivas sólidas para atenuar as sequelas motoras permanentes. O estudo, liderado pela investigadora Sara Meirinho, contou com a publicação na revista especializada *Drug Delivery and Translational Research*.

A cooperação multidisciplinar permitiu desenhar ensaios rigorosos de autoemulsificação para transporte do composto activo. Por outro lado, a equipa sublinha que a solução não elimina as causas de base da patologia, mas protege a estrutura cerebral. Trata-se de um avanço terapêutico relevante desenhado a partir dos laboratórios da região centro.

Ensaios clínicos e potencial alargado a outras doenças

Apesar do entusiasmo com os resultados, a equipa de cientistas mantém uma postura de prudência. O trabalho encontra-se ainda na fase pré-clínica, pelo que faltam etapas essenciais antes de avançar para testes em humanos. Com efeito, serão necessários novos estudos de toxicologia e ensaios clínicos controlados para atestar a segurança e a dosagem exacta em doentes.

Além disso, o potencial desta descoberta vai muito além do acidente vascular cerebral. Os investigadores sustentam que a via de administração e o efeito neuroprotector podem servir para tratar outras patologias graves do sistema nervoso central. Entre as patologias prioritárias para futuros ensaios laboratoriais encontra-se a esclerose múltipla.

Deste modo, a universidade portuguesa reafirma a sua capacidade competitiva à escala internacional. O consórcio prepara agora a validação regulatória da tecnologia. O objectivo passa por captar novo financiamento comunitário e transferir esta solução terapêutica para a prática hospitalar corrente.
